LIST OF VALID ID’s
1. Passport
2. Driver’s License
3. Professional Regulation Commission (PRC) ID
4. National Bureau of Investigation (NBI) Clearance
5. Police Clearance
6. Postal ID
7. Voter’s ID
8. Barangay Certification or Barangay IDs or similar documents bearing picture of the member
9. Government Service Insurance System (GSIS) e-Card
10. Social Security System (SSS) Card
11. Senior Citizen Card
12. Overseas Workers Welfare Administration (OWWA) ID
13. Overseas Filipino Worker ID
14. Seaman’s Book or Seafarer’s Identification and Record Book (SIRB)
15. Alien Certification of Registration/Immigrant Certificate of Registration
16. Government Office and GOCC ID, e.g. AFD ID, Pag-IBIG Loyalty Card
17. Certification from the National Council for the Welfare of Disabled Persons (NCWDP)
18. Department of Social Welfare and Development (DSWD) Certification
19. Integrated Bar of the Philippines ID
20. Company ID issued by Private Entities or Institution Registered with or supervised or regulated either by the BSP, SEC or IC.
PROCEDURE:
STEP 1
-
Submit accomplished Application for Provident Benefits (APB) Claim Form and supporting documents.
STEP 2 Get Provident Benefits Acknowledgment Receipt (HQP-PFF-061).
STEP 3 On scheduled date, get claim proceeds.

NOTES:
a. If Pag-IBIG Loyalty Card is not available, two (2) valid IDs (present original and submit photocopy).
b. For retirement purposes, the valid IDs must reflect the member’s date of birth. If the valid IDs do not reflect the date of birth, submit any of the following:

Birth Certificate of Member issued by Philippine Statistics Authority (PSA)

Non-availability of Birth Record issued by PSA and Joint Affidavit of Two Disinterested Persons (notarized)
