    2) 2 valid government issued IDs of borrower/s

    3) PSA Marriage Certificate and 2 valid government issued IDs of Spouse, if married

    4) Latest Statement of Account from developer

    INCOME REQUIREMENTS

    IF OFW:

    1) Duly signed and accomplished AIF form (refer to attached)

    2) 2 valid government issued IDs of attorney-in-fact

    3) Latest Certificate of Employment with salary, job position, and tenure

    4) 3 months latest payslips

    5) 3 months latest bank statements of payroll account

    6) Oath of allegiance as proof of dual citizenship for former Filipinos

    7) Latest Federal ITR (for US/Canada Based borrowers)

    8) Passport ID with all copies of arrival and departure stamps to and from the Philippines

    IF LOCALLY EMPLOYED:

    1) Latest Certificate of Employment with salary, job position, and tenure

    2) 3 months latest payslips

    3) Latest ITR

    IF SELF-EMPLOYED:

    1) DTI Registration and Mayor's Permit (if Single Proprietorship)

    2) Latest GIS / Articles of Incorporation and By-Laws / SEC Registration (if Corporation)

    3) Latest ITR with audited financial statements

    4) 6 months latest bank statements of business' working account

    5) List of major clients and/or suppliers of business (answer page 2 of attached authorization letter)

    If business is Corporation and borrower has an individual income from the business, to submit the following requirements:

    7) Latest Certificate of Employment with salary, job position, and tenure

    8) 3 months latest payslips

    COLLATERAL REQUIREMENTS
